Patient Engagement Portals

We’re currently working with trusts and Integrated Care Systems through their Patient Engagement Portal projects.

We are supporting East & North Hertfordshire NHS FT with SME project and programme management services to safely manage the technical development and roll out of their solution.

The Result

Evolution, a leading provider of digital transformation services, played a crucial role in supporting the East and North Hertfordshire NHS Trust (E&NH) with centralising their outpatient transformation efforts.

The project focused on the Trust’s Patient Engagement Portal (PEP), a digital platform designed to enhance patient experience and streamline communication between healthcare providers and patients.

The scope of work included, but was not limited to:

  • Digitisation of Letters
  • Patient Engagement Portal (PEP) Roll-out
  • Centralisation of Outpatient Services
  • Letter Template Standardisation
  • Constellation Management

Throughout the project, Evolution’s Digital Change Lead has worked closely with E&NH’s staff and leadership to ensure a smooth transition, provide training and support, and continuously monitor and optimise the centralised outpatient transformation process.
