Hertfordshire Partnership NHSFT (HPFT) approached us with a remote care project. In the early stages of the pandemic, they had vulnerable patients who needed regular monitoring but didn’t necessarily need to come into clinic. HPFT secured a blue box solution from a technology vendor whereby monitoring key health measures such as glucose, oxygenation and blood pressure can be completed remotely. HPFT had a plan to roll out the blue box solution to people in care homes initially, knowing that care professionals were available who could help with deploying and roll out. HPFT used this model as a Proof of Concept and blueprint to further roll it out to wider care settings across the region.
The Trust needed a team to review and assure the technology, develop a robust but rapid deployment and training plan, report on results, collect and collate data, and deal with change. Although HPFT has a dedicated digital team and clinicians, they didn’t have the necessary capability or capacity to bridge the gap between the technology vendor and the trust, therefore, Evolution were responsible for; driving the project through the delivery phases; building the evidence pack; identifying, managing and mitigating risks; training and change management; and ongoing support.
Evolution have a strong track record in the delivery of process redesign and transformation initiatives, particularly within the health sector. Our team at Evolution regularly partner with solution providers and NHS organisations to drive sustainable outcomes across a multitude of digital; and technology initiatives. We worked with them to develop a proposal to deliver the project from start to finish. We provided analysis to determine which parts of the project the technology vendor, AI Digital, could carry out with their existing team and complemented that with a team of subject matter experts using an agile methodology, allowing for a lean, efficient way of completing HPFT’s goal. By working with AI Digital through to the completion of the project, we ensured that the Trust avoided any unnecessary overspend and completed the roll out in tight timescales and within budget.
Evolution delivered the project according to plan, carrying out training, implementing the remote care technology, and carrying out post-implementation support, and building and maintaining a document configuration library including blueprints. Following this, we were retained to carry out the second roll out with two different groups over a 12 month period. For many projects where there isn’t the internal capacity to complete them, bringing in day rate contractors to carry out the work isn’t practical. For example, in this case, a trainer was required for approximately eight days across six months. Our solution was able to deliver all of the work, take ownership for the project and ensure that it was fully-staffed (without over-staffing) whilst also working completely in partnership with the Trust.
